Purpose:

The Occupational Therapist is a member of the interdisciplinary team providing pre and post transition services to people with mental and/or physical illness who are part of the Williams or Colbert Class Membership. Some Class Members may also have a secondary diagnosis of substance abuse, intellectual and/or developmental disabilities, or other clinical issues. The OT will provide individual and group interventions to Class Members in long term care facilities to help prepare them for transitioning to the community. The OT will also work with Class Members post-transition to help them better integrate into the community. The OT will work with Class Members to help them develop, maintain, or restore physical and mental functioning through engagement in meaningful and productive occupations, including ADLS, IADLs, rest and sleep, education, work, play, leisure, health management, and social participation. The OT will help increase a Class Member's ability to live as independently and safely as possible in the community.

Essential Responsibilities, include but are not limited to:

• Provide compassionate care and recovery-oriented services to Class Members with mental illness and/or physical disabilities.
• Evaluate a Class Member's performance of ADLs, IADLs, and functional mobility.
• Assess personal, environmental, medical, and social variables that influence a Class Member's ability to do what they want and need to be able to do consistently, safely, and with adequate skills.
• Assess a Class Member's ability and capacity to transition, and make recommendations for supportive services and the most appropriate community setting.
• Develop and implement a treatment plan based on needs and Class Member's goals, to support successful community transition.
• Assist Class Members in achieving independent living goals by providing skill building and education in various areas, such as ADLs, mobility/community mobility, medication management, meal preparation, budgeting, hobbies/leisure activities, grocery shopping, transportation, and social participation.
• Attend meetings and consult with team regarding Class Member's progress and share recommendations.
• Collaborate with interdisciplinary professionals (i.e. Physicians, Therapists, Psychiatrists, Nurses) for comprehensive care to support community living.
• Comply with assessment, treatment, and documentation guidelines from Envision Unlimited, DHS, and the IL OT Practice Act.
• Perform other related duties or projects as assigned.

EDUCATION, SKILLS, and/or EXPERIENCE:

• Master's degree in Occupational Therapy w/ license to practice in Illinois.
• Prefer someone with experience but will consider internships for new graduates.
• Must have valid drivers license
• Must be able to pass background check.

Compensation & Benefits:

• Base Salary: $80,000 per year
• Paid Time Off: 11 Vacation days, 12 Holidays days, 3 sick days, 3 personal days
